ACEIs ( NAPLEX Review)
- Recent guidelines recommend the use of ACEIs in many types of patients with HF owing to left ventricular systolic dysfunction unless the patients have a contraindication to their use or have demonstrated intolerance to their use.
- ACEIs received a class IIa recommendation with a level A level of evidence for prevention of HF in patients at risk, stage A, who have atherosclerotic vascular disease, diabetes mellitus, or hypertension with associated cardiovascular risk factors.
- ACEIs received a class I recommendation with a level A level of evidence for use in all patients with a recent or remote history of MI, stage B, regardless of ejection fraction or presence of HF, and a class I recommendation with a level A level of evidence for use in patients with a reduced ejection fraction and no symptoms of HF, even if they have not experienced an MI.
- ACEIs received a class I recommendation with a level A level of evidence for use in all patients with current or prior symptoms of HF (stage C), and reduced LVEF, unless contraindicated.
- ACEIs did not receive a recommendation for use in HF patients with stage D due to the need for special procedures, such as transplantation, implantable pumps, etc.
- ACEIs did receive a class IIb recommendation with a level C level of evidence for use in HF patients with normal LVEFs.
