Administer N-acetylcysteine (Acetadote®)
- N-acetylcysteine is a precursor for glutathione and decreases APAP toxicity by increasing glutathione hepatic stores. It does not reverse damage to liver cells that have already been injured.
- Ideally begin treatment within 8 hrs of ingestion. It may be of value 48 hrs or more aft er ingestion.
- Adult oral dose: 140 mg/kg, followed by 70 mg/kg every 4 hrs times 17 doses.
- Adult IV dose: 150 mg/kg infused over 60 mins, followed by 50 mg/kg infused over 4 hrs, followed by 100 mg/kg infused over 16 hrs.
